Atrium operates under HIPAA as a closed-record practice. We share information only with member written consent or as required by law. We never sell or share data with marketing companies. Our full Notice of Privacy Practices is provided at the first visit and available on request.

RIGHTS

Patient + member rights.

You have the right to access your medical records, request amendments, request a confidential channel of communication, request restrictions on disclosures, and request an accounting of disclosures. Routine requests processed within 30 days; complex requests within 60.

MEMBERSHIP TERMS

Membership agreement.

Membership is month-to-month or annual at member's choice. Cancellation requires 30 days written notice; annual prepay refunds the unused balance. Membership confers no right to any specific physician's panel; members may transfer between physicians with mutual consent. Full terms provided at the first visit.
