Medicare
We participate with original Medicare. Part B typically covers cardiology consultations and the diagnostic tests we perform, subject to your deductible and coinsurance.

Medicare Advantage
Most regional Advantage plans include us in network; many require a referral from your primary-care physician — we'll confirm your plan's rule when we verify benefits.

Commercial plans (PPO / HMO)
We participate with the major regional commercial networks. HMO plans usually need a referral on file before the visit; PPO plans usually don't.

Self-pay
No insurance, or prefer not to use it? We keep a simple flat-fee schedule (below) and provide a written good-faith estimate before any visit or test.
How verification works here
When you book, June checks your benefits — network status, referral requirements, deductible position — and calls you before the visit if anything needs attention. If a test requires prior authorization, we obtain it before scheduling the test, not after performing it. Plan networks change; if you're unsure about yours, call (555) 014-7821 and we'll check together.
Self-pay
Flat fees, published on purpose
For patients without insurance — or who prefer not to use it — visits and tests are flat-fee, due on the day, with a written good-faith estimate beforehand. Sample figures for this fictional demonstration practice.
