Insurance and billing
Clear answers about insurance and cost
We bill Medicare, Wisconsin Medicaid and most large plans. If you're paying yourself, our prices are posted below, and we'll always tell you before a visit changes what you pay.
Plans we bill
This is a plain list of the plans we send claims to. It isn't a list of partners, and it doesn't guarantee your specific plan is in network.
Before your first visit
Call the number on your insurance card and ask if Larkfield and your clinician are in network. Or call us and we'll check together.
- Medicare (Original Medicare, Part B)
- Medicare Advantage plans from Aetna, Humana and UnitedHealthcare
- Wisconsin Medicaid (BadgerCare Plus)
- Aetna
- Cigna
- UnitedHealthcare
- Blue Cross Blue Shield plans
- Marketplace (ACA) plans: ask us about your network

Preventive visit or problem visit?
This is the most common billing surprise in primary care, so here it is in plain words.
Preventive visit
Often no copay with an in-network plan
- Your yearly physical or wellness visit
- Screenings you're due for, like cholesterol or a Pap test
- Recommended vaccines
- Counseling on things like diet, smoking or alcohol
Problem (office) visit
Your usual copay or deductible applies
- New symptoms, like a cough, rash or knee pain
- Managing an ongoing condition, like diabetes or blood pressure
- Changing or refilling medicines for a condition
- Tests to look into a symptom or an unusual result
Our promise: if something you raise at a physical would turn part of the visit into a billed problem visit, your clinician tells you first. You can go ahead, or book it as a separate visit.
Self-pay prices
For patients without insurance, or who choose not to use it. Payment is due at the visit.
| Visit or service | Price |
|---|---|
| New patient visit (40 minutes) | $180 to $260 |
| Office visit, current patient | $110 to $170 |
| Adult physical | $195 to $275 |
| Video visit | $75 to $110 |
| Sports or school physical | $75 |
| Flu shot | $40 to $55 |
| Lab draw fee (lab bills its tests separately) | $15 |
Prices are sample ranges for this demonstration site. The exact price depends on the length and complexity of the visit.

Billing questions
How do I know if you're in network for my plan?
Call the member number on your insurance card and ask whether Larkfield Family Medicine and your chosen clinician are in network, or call our billing team and we'll check with you. Plans change networks every year, so it's worth checking each January.
Why did my physical come with a copay?
A preventive visit is usually covered in full. If a new or ongoing problem was also treated, your plan may bill that part as an office visit. We tell you during the visit before that happens, so you can choose to book the problem separately.
Do you offer payment plans?
Yes. If a bill is more than you can pay at once, call our billing team and we'll set up monthly payments with no interest.
What is a Good Faith Estimate?
If you don't have insurance, or choose not to use it, you have the right to a written estimate of expected charges before a scheduled visit. Ask when you book, and we'll send one.
Insurance sorted? Book your visit.
Tell us your plan in the booking form and we'll check your coverage before the visit. If anything looks off, we call you first.
