Accepting new patients

New to Larkfield? Here's how to start

Switching doctors takes one booking and one signature. We request your records, send your forms ahead of time, and give you a longer first visit to get to know you.

Five steps from first call to first visit

  1. Step 1: Pick a clinician (or let us)

Choose anyone marked “Accepting new patients”, or pick First available and we'll match you by clinic, schedule and what you need.

  2. Step 2: Book a 40-minute first visit

Online or by phone. First visits are longer than follow-ups so there's time for your history and your questions.

  3. Step 3: Sign the records release

It comes with your confirmation. We use it to request your records from your old office, so you don't have to chase them.

  4. Step 4: Fill in your forms at home

The patient portal sends intake forms a few days before. About fifteen minutes, and you can save and come back.

  5. Step 5: Come in and meet us

Arrive ten minutes early to check in. You'll leave with a written plan and a follow-up booked if you need one.

What to bring to your first visit

For adults

  • Photo ID and your insurance card
  • Every medicine you take, including vitamins and supplements (bottles are easiest)
  • Names of your past doctors and any specialists
  • Recent lab results or imaging reports, if you have copies
  • Your vaccine record, if you have one
  • A short list of questions or concerns

For children

  • Your child's vaccine record
  • Their insurance card and your photo ID
  • Birth hospital name and any newborn screening results
  • School, daycare or sports forms that need signing
  • Names of any specialists or therapists they see
  • A favorite toy or snack for little ones

How we transfer your records

Your records belong to you, and moving them is our job. Here's what happens after you sign the release.

  1. You sign one release form

It names your old office and lets them send your records to us. Sign it online with your confirmation, or at check-in.

  2. We send the request

Our records team sends it to your old office by secure fax or electronic exchange, usually within one business day.

  3. Your old office sends the records

Most offices send them within a couple of weeks. Some charge a small copy fee, and we'll tell you if they do.

  4. Your clinician reviews them

Important history, recent labs and your vaccine record are added to your Larkfield chart before or soon after your first visit.

Did your old office close? Practices that close must arrange for someone to keep their records. Tell us the clinic name and we'll help you track down who has them.

New patient questions

Are you accepting new patients?

Yes, at both clinics. Most of our clinicians are accepting new patients of all ages, and Dr. Raman is accepting family members of her current patients. The Providers page shows each clinician's status.

Do you take my insurance?

We bill Medicare, Wisconsin Medicaid, and most large commercial plans. See the Insurance page for the list of plans we bill. Networks change every year, so call us or your plan to confirm before your visit.

How do I move my records to Larkfield?

Sign our records release form (we send it with your appointment confirmation), and we request your records from your old office. You're welcome to bring copies of recent labs, your vaccine record and your medicine list too.

Will I see the same clinician every time?

You'll have one primary clinician who knows your story. When they're away or booked up, a teammate sees you and sends them a note, so nothing gets lost.

What if I don't have insurance?

Our self-pay prices are listed on the Insurance page. If you're paying yourself, you can ask for a Good Faith Estimate of costs before your visit.

Can I bring someone to my visit?

Yes. A family member, friend or caregiver is welcome in the room, and we'll always ask you privately if there's anything you'd like to discuss alone.
