Only if you want it to. For health-related or otherwise private appointments, we write the reminder to confirm a time without describing the visit, so it doesn't disclose anything to anyone but the patient.
Depending on your booking system's capabilities, yes, some support a reply-to-confirm or a link to reschedule. Where that's not available, the reminder gives a number to call instead.
